Section 9202 of the consolidated OMNIBUS Budget Reconciliation Act of 1985 required the
Centers for Medicare & Medicaid Services (then known as the Health Care Financing
Administration), to establish a unique physician identifier for each physician and or
non-physician billing Medicare Part B.

As such, the Unique Physician Identification Number (UPIN) was born. The UPIN
is a 6-digit, alpha numeric identification number issued by the UPIN Registry.
NHIC, Corp. currently administers the Registry.

ABOUT NHIC, Corp. |

NHIC, Corp. was founded in
1976 as a wholly owned subsidiary of EDS Corporation.

On December 1, 1996, NHIC, Corp. became the CMS Contractor
for Northern California.
|
 |
In August 1997, NHIC, Corp. assumed the replacement Contractor for Medicare Part B in
Maine, Massachusetts, New Hampshire, and Vermont.

On December 1, 2000 NHIC, Corp. became the Medicare Part B
Contractor for seven Southern California counties, making
NHIC, Corp. the first Contractor in 30 years to process all of
California's Medicare Part B claims.

Additionally, on December 1, 2000, NHIC, Corp. became the national
Registry Contractor responsible for administering the UPIN
program for CMS
